This must match your identity document so admin can verify your application quickly.

IC Number *

Use the standard Malaysian IC format. This helps prevent duplicate therapist registrations.

Gender *

Required for therapist profile setup and service matching.

Years Experience *

Tell us your real experience level so customers see the right profile details.

WhatsApp Number *

Use Malaysian format, for example +60123456789. This is how admin and customers will reach you after approval.

Public Profile

Display Name *

This is the only name customers will see on the app, so choose the public name you want to be known by.

Short Bio *

Write a short introduction about your care style, strengths, or the clients you serve best.

Languages Spoken *

Malay English Mandarin Tamil Arabic Other

Select at least one language so customers know how you can communicate during sessions.

Logistics

Full Address *

Enter your therapist base address clearly. We use this for matching, route calculation, and the 20km service radius.

Verification Uploads

IC Photo (front only) *

Upload a clear front image of your IC only. Make sure the text is readable and not cropped.

Professional Certificate *

Upload your main professional certificate clearly. A clean PDF or photo works best for admin review.
